● Service Users: Personal identifiers, contacts and characteristics (e.g., name, contact
details and medical history).
●Families and Friends: Details of next of kin and family members to contact in case of
emergency (e.g., home address, telephone number, email address).

● Most of the personal information we process is provided to us directly by you for one

● For care needs assessment
● In case of an emergency
●To work with other healthcare professionals already working with you.
We also receive personal information indirectly from the following sources in the
following scenarios:
● Social Workers
● Hospital Discharge team
● GPs
● District Nurses
● Local Authorities
● Families
We use the information you gave us to provide your care needs, such as your key safe
code, which we have to share with the carer looking after you.
We may share this information with your social worker, paramedics, district nurses,
physiotherapist, family of your choice, and doctor and GP surgery.
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely

Your information is securely stored on our system, both soft and hard copies.We keep
your Personal Information for the period we provide care for you, and we will then
dispose of your information by the end of your service for your care with us.

Your information that is stored on our system will be deleted, and we shred the
hardcopy documents.
